Ready-made downloadable forms to just print and use as you need them. Having a written contract with parents is highly recommended. This helps to set out all the agreements with each party having a copy to refer to, which avoids the possiblity of misunderstandings.

Contracts must be completed and signed by the Parent/Carer(s) who have Parental Responsibility for the child. In order to comply with the Data Protection Law, you must make all reasonable efforts to verify that each person giving consent does, in fact, hold parental responsibility for the child.
