This pack gives you everything you need to ensure you are meeting legal requirements by recording any accidents, injuries or incidents that may have occurred to a child. We have included a form where you can record both permission and details of medication administered to a child. Finally there is a form to record any safeguarding concerns you may have about a child.

Childminding UK have put together 3 separate forms which together make up this Accident, Injury and Medication Pack to make sure no essential information is missed. These forms can be downloaded and printed for your convenience. They are important documents to ensure you are meeting all the requirements of the EYFS.
It is good practice to ask parents to sign the entries made in these forms. Records should be kept for a reasonable amount of time after the child leaves your care as stated in the EYFS. It is recommended that medication records are kept until the child reaches 21 years of age.
