Childminding settings are usually very safe, but in the case of an emergency where the childminder may need to take a child to the hospital for emergency treatment for example, it is helpful to have planned who will take care of the minded children until their parents can arrive to collect them.
This form enables you to record parents permission to share their contact details with these emergency back-up people. These people may be other registered childminders or other people that you trust, that the child knows and who are likely to be available.
